Thank you for your patience with our medical record personnel changes and office relocation. There are a few things to let you know before you come!
- **I will be contacting those who requested records after I have them completed them and they are ready for pick up. If I have not contacted you, or if there are records for more than one person in the household I have contacted, please make sure to email the office before arrival to assure all records are ready. Ring the doorbell if the door is locked. **

Due to HIPPA restrictions, you must present a photo ID in order to leave with your records.

If you are authorized to pick up records for another patient, you must bring a signed and dated note that says so (In addition to your ID).
